Based on the findings of an onsite unannounced hospice agency state re-licensure survey conducted on June 9, 2021 through June 11, 2021, Holisticare Hospice, LLC, was found to be in compliance with the requirements of 42 CFR, Part 418, Subparts A, C, and D, Conditions of Participation: Hospice Care.
